Inifrånperspektivet om hur det är att leva med dissociation eller en dissociationsrelaterad diagnos är underrepresenterad i all klinisk utvärdering och dessa personers upplevelser och
åsikter måste tas i beaktande för att vården som erbjuds ska vara meningsfull och återhämtande (McAllister et al., 2008). Enligt kompetensbeskrivningen för
specialistsjuksköterskor inom psykiatrisk vård har specialistsjuksköterskan ett ansvar att
”utifrån ett personcentrerat förhållningssätt tillvarata patientens och närståendes
erfarenheter, behov, mål och preferenser i planering och genomförande av vård och omsorg”
samt ett ansvar för att personer med psykisk ohälsa erbjuds vård på lika villkor (Psykiatriska riksföreningen för sjuksköterskor, 2014, s.8).
Denna studie visar att personer som lever med svår dissociation har många svårigheter och mycket smärta att hantera. Att till exempel bli inlagd kan både vara mycket stressande och energikrävande, med en ökad mängd dissociativa symtom som följd, men om vårdarna har kunskap om dissociation och har bättre förståelse vad dessa personer går igenom inombords i form av motstridigheter, smärta, tvivel, svårigheter att lita på andra m.m. så kan det göra situationen betydligt mindre stressfylld, smärtsam och traumatisk. Det kan också vara så att inläggningen kan vara det tillfälle som ger dem rätt diagnos. Vi tänker att mer kunskap och förståelse alltid är positivt, om denna uppsats kan göra så att, om så bara en person får en mer positiv upplevelse och bemötande av vården så är det värt det. Det är inte lätt att leva med en kontroversiell diagnos, att stå upp för sig själv om en råkar stöta på en av de inom vården som kanske inte ens tror på att diagnosen existerar och inte har rätt kunskap och förståelse för vad personen varit med om och vilka svårigheter som personen upplever i det dagliga livet. Med en förståelse kan en verkligen göra skillnad för personens möjligheter till återhämtning.
Slutsats
Denna uppsats tyder på att personerna med svår dissociation upplever att de är i behov av trygghet, sociala kontakter, acceptans och professionell hjälp av personer som bekräftar, är närvarande och som har kunskap om dissociation, för att få deras liv och vardag att fungera.
Det finns också en betydande vilja att få leva ett liv med en fungerande vardag. Det finns dock stora hinder med den rådande stigmat runt sexuella övergrepp mot barn och den bristande kunskap om DID och andra dissociativa diagnoser, i den värld vi lever i.
Det är svårt nog för personerna att öppna upp och be om hjälp, och med allmänhetens bild som färgats av media, och den tabu som råder rörande incest och sexuella övergrepp på barn, gör det inte situationen lättare för personerna. Specialistsjuksköterskor och annan
vårdpersonal som möter dessa patienter behöver ta dessa personer och deras röster på allvar om vi ska leva upp till vårat ansvar av att erbjuda dessa patienter en väg mot återhämtning, välbefinnande och livskvalitet. Vi anser att alla inom vården bör ha en viss insikt i
psykiatriska diagnoser, då dessa patienter inte bara finns inom psykiatrin. Som sjuksköterskor inom psykiatrisk vård anser vi att alla har ett egenansvar att uppdatera sin kunskap om de dissociativa och traumarelaterade diagnoserna, etiologin och orsaken bakom dem, hur det dagliga livet påverkas vid svår dissociation och vad en ska tänka på i mötet med dessa patienter. Det behövs vidare forskning som belyser personers egna upplevelser och erfarenheter av att leva med svår dissociation och DID.
