Consent to vaccination against Covid-19
Information and offer of vaccination against Covid-19
There are vaccines against Covid-19 available now which have been approved for children and whose benefits are well known. The Public Health Agency of Sweden recommends universal vaccination against Covid-19 from the age of 12, beginning on 11 October 2021. The recommendation refers to primary vaccination with two doses of mRNA vaccine. The decision by the Public Health Agency of Sweden is based on the best interests of the child, in which the benefits of vaccination are deemed to outweigh any risks for the individual child.
Vaccination against Covid-19 is voluntary and free of charge in Sweden for anyone who wants to get vaccinated. You can use this form to notify if you want your child to get vaccinated or not by checking one of the boxes below.
If guardians do not agree or if consent is not given means that your child will not get vaccinated, unless they want to get vaccinated and they are deemed to have reached such maturity that they can consent to the vaccination. For full protection, the vaccine is administered twice with an interval of several weeks, more information will be given at the time of vaccination. The consent covers both doses.
Prior to the vaccination we would ask you to please answer the questions in the attached health declaration regarding your child Health declaration - Hälsodeklaration inför vaccination mot covid- 19.pdf (vgregion.se)
A new health declaration needs to be filled out prior to the second dose of the vaccination.
