Syftet med studien var att ta reda på vilka krav som ställs på en digital plattform då akutjournalen som används på akutmottagningen vid Universitetssjukhuset i Linköping ska överföras till en sådan. Nedan presenteras slutsatser från studien.
Hur ser arbetsflödet med akutjournalen på akutmottagningen vid
Universitetssjukhuset i Linköping ut idag?
Det studien kom fram till var att akutjournalen passerar en mängd olika människor och yrkesroller. Den börjar hos en läkarsekreterare, hamnar sedan hos ledningssjuksköterskan vilken delar ut den till ett team. Där tar en sjuksköterska eller undersköterska över den för att triagera patienten. Därefter tar en läkare akutjournalen för att träffa patienten och ordinera läkemedel. Sedan cirkulerar akutjournalen mellan sjuksköterska, undersköterska och läkare i teamet medan dessa vårdar patienten. När patienten är tillräckligt frisk för att åka hem eller till en avdelning på sjukhuset återgår akutjournalen till läkaren som dikterar dess innehåll. Till sist får en läkarsekreterare åter akutjournalen och avlyssnar diktatet, skriver in det i COSMIC, skannar in akutjournalen och slänger den sedan.
Vilka kvaliteter har akutjournalen som bör bevaras vid överföring till en
digital plattform?
En hel del kvaliteter identifierades med pappersformatet, såsom att det är en artefakt som är lätt att bära på och flytta runt. Pappersformatet möjliggör även för alla yrkesroller att dokumentera i samma akutjournal, den ger en bra överblick och att få ett papper i handen gör det tydligt för den sjuksköterska eller undersköterska som tar emot det att dennes team har fått en ny patient.
Vilka svagheter finns med akutjournalen som kan lösas vid överföring till
digital plattform?
Några av de problem som identifierades med akutjournalen var att det sker en del dubbeldokumentation då en del av informationen i akutjournalen även ska dokumenteras i COSMIC, det glöms bort att fylla i att en patient har fått in- och utfarter uttagna och det finns inte tillräckligt med utrymme för att skriva in nya vitalparametrar.
Alla kvaliteter och problem, vilka finns beskrivna i de tre första delarna under rubriken Krav, i resultatkapitlet, ledde fram till en kravspecifikation av vad en digital plattform bör innehålla om akutjournalen ska överföras till en sådan.
