1. This report must be filed between January 1 and May 1 of each year by every nonprofit corporation

incorpor-ated in Colorado. The Secretary of State is required by law to RETURN every report which does not conform to the

Colorado Statutes- i.e. if the Report contains an INCOMPLETE OR INCORRECT PARAGRAPH.

2. Every nonprofit corporation in Colorado is required to designate a registered office in Colorado and a registered agent at such office.

3. All information shall be CURRENT.

4. This report must be accompanied by payment of a filing fee of $5.00 payable to the Secretary of State of

Colorado.

5. The LAW makes NO PROVISION for an EXTENSION OF TIME for filing Annual Report.

6. Annual Report becomes DELINQUENT May 2nd. A PENALTY of $10.00 will be assessed after May 1sr.
